Participant Information Sheet - Smart Recovery International Attitudes Towards Tobacco Treatment Survey
(Parent Study: Evaluating the SMART Recovery National Data Collection Project)

Who is doing the study? 
Dr Peter Kelly at the School of Psychology, University of Wollongong, Australia is leading the study. Researchers involved in the study are Dr Kristen McCarter (University of Newcastle), Ashleigh Guillaumier (Flinders University), Prof Amanda Baker (University of New South Wales), Associate Prof Victoria Manning (Monash University), Associate Professor Brian Hitsman (Northwestern University) and Ed Day (University of Birmingham).
What is the study about? 
The aim of the study is to examine the attitudes and perspectives of SMART Recovery facilitators towards addressing smoking cessation as part of SMART Recovery. The survey includes questions about your background as a SMART Recovery facilitator and your attitudes and opinions regarding addressing tobacco smoking within SMART Recovery. The study is for people that are currently or have facilitated a SMART Recovery group in the last 5 years. 
What do I need to do? 
If you would like to participate, please complete the survey below. It will take approximately 10 minutes to complete.
Do I have to participate in the study? 
Participating in this study is entirely voluntary. If you choose not to participate, doing so will not affect your relationship with SMART Recovery or the University of Wollongong. If you decide to take part in the study and then change your mind later, you are free to stop participating at any stage or refuse to answer any of the questions. However, it will not be possible to withdraw your individual data from our records as no identifying information is collected as part of this study and your responses will not be identifiable (i.e. anonymous).
Is there any risk or burden if I decide to participate? 
We do not anticipate any risks or additional burdens associated with participating in the study. The information entered into the survey does not include any identifying information for individual participants attending your groups. 
Are there any benefits to be expected? 
The purpose of this survey is to help inform possible implementation of tobacco treatment support within SMART Recovery, if found to be acceptable and feasible. 
How will my information be collected and used? 
The data that you enter will be made available to the research team. This information will be collated with data from other facilitators. Journal articles, conference presentations, or internal reports may be developed from this study. However, these publications will not identify individual facilitators. Rather, it will provide group based analysis of the findings.
