Principal_School Consent

To provide your consent, please review the following points before clicking agree in the box below:

I consent to the teachers and school staff from my school participating in an education intervention including completion of the teacher food and nutrition questionnaire at three time points, before, immediately after program completion and at three months follow up.

I understand that the project will be conducted as described in the Participant Information Sheet, a copy of which I have read and retained.

I consent to include and schedule within a regular teacher meeting in term 3, a short 15-20 minutes face-to-face information session of which all school staff including teachers, executive and administrative personnel will be invited to attend, in a voluntary capacity.

I consent to complete two questionnaires before and following program completion (Approximately 30 minutes each).

I understand that the program will go over a four-month period comprising of the following:

I understand that my school’s participation in this research is voluntary, and that I can withdraw organisational consent at any time.

I understand the participation of my staff is also voluntary, that they can withdraw from the project at any time and do not have to give any reason for withdrawing.

I understand that the personal information of participating teachers will remain confidential to the researchers except as required by law.
